We Know This Moment Is Hard.

You Don't Have to Navigate It Alone.

Whether you’re planning ahead or facing an urgent transition, Hope Hearth is here to walk with your family — not rush you through a process. We’ll take the time to understand your loved one as a person before we talk about services.

"My Parent Can No Longer Care for My Sibling"

When an aging caregiver’s health changes, the family faces an urgent need for residential support. Hope Hearth specializes in welcoming adults with IDD into stable, loving home environments during this transition. Your loved one’s routines, preferences, and personality will guide everything we do.

"My Child Is Aging Out of School Services"

The transition from school-based supports to adult services is one of the most stressful experiences families face. Hope Hearth can provide in-home support, respite, and — when the time is right — residential placement that gives your young adult room to grow while keeping them safe.

"I Need a Break — I'm Burning Out"

You are not failing by asking for help. Our respite services give you time to rest, recharge, and take care of yourself — while someone trained and trustworthy is with your loved one. We come to your home, learn your routines, and make the transition seamless.

"My Loved One Wants to Leave a Facility"

Through the Money Follows the Person program, Hope Hearth helps individuals transition from nursing facilities, group homes, and institutions into community-based homes where they can live with independence, choice, and dignity. We handle the transition logistics so your family can focus on what matters.

Your Situation

Find Your Path

What to Expect

We Start by Listening

We have a conversation

Not an intake form — a conversation. What does your loved one enjoy? What makes them anxious? What does a good day look like?

We explain your options clearly

We’ll help you understand which services the waiver covers, what we provide today, and the timeline. No jargon, no pressure.

We coordinate with your case manager

If we’re the right fit, we develop a person-centered plan reflecting your loved one’s goals and preferences — not ours.

We introduce our team

Before services begin, you’ll meet the DSPs who will provide support. Trust is built face-to-face, not through paperwork.

Your Rights

Your Loved One's Rights Are Non-Negotiable

At Hope Hearth, every person we support has the right to privacy in their own space — including a lockable bedroom door. The right to choose their own daily schedule. The right to eat what they want, when they want. The right to welcome visitors at any time. The right to select their own providers. And the right to be treated with dignity, respect, and freedom from coercion at all times.

These aren’t aspirational values. They are federal HCBS requirements, and they are embedded into every policy, every training, and every interaction at Hope Hearth.
