Community-Based Residential Living
CIH Residential Habilitation and Support (Daily) · All Levels
"A Home. Not a Facility. Yours."
Hope Hearth operates small residential homes in regular neighborhoods throughout Central Indiana, each serving 2–4 adults with intellectual and developmental disabilities. Our homes are designed for community integration — located on quiet streets with neighbors, parks, shops, and public transportation nearby.
Each person in our care has their own private bedroom with a lockable door. They choose how to decorate their space, set their own daily schedule, access the kitchen whenever they’re hungry, and welcome visitors whenever they wish. Our trained Direct Support Professionals provide 24/7 support — not supervision. They’re there to help each person build skills, maintain health, pursue interests, and participate in the community on their own terms.
Who this serves:
Adults with IDD who meet CIH waiver eligibility — including individuals whose aging caregiver can no longer provide support, individuals transitioning from institutional settings, and individuals with complex support needs requiring a structured but home-like environment.

"Because Caregivers Need Care, Too"
Family caregivers are the backbone of Indiana’s support system for people with IDD — and they deserve rest. Hope Hearth provides planned and emergency respite services, delivered by trained professionals who take the time to learn your loved one’s routines, preferences, and personality before the first visit.
Respite can be provided in the individual’s home or in a Hope Hearth residential setting. We offer daytime, evening, overnight, and weekend availability — because caregiver burnout doesn’t follow business hours.
Available under both the CIH Waiver and Family Supports Waiver (FSW)

"Support That Comes to You"
For individuals living at home with family or independently, our Participant Assistance and Care service provides direct support in the individual’s own environment. Our DSPs help with daily living skills, community access, social engagement, and personal care — always following the individual’s lead about what support looks like for them.
PAC is built around flexibility. Need help three mornings a week with routine and meal prep? We’re there. Want someone to accompany you to a community event on Saturday? We’ll go together. The schedule, the goals, and the pace are set by the individual and their family — not by us.

For individuals living at home with family or independently, our Participant Assistance and Care service provides direct support in the individual’s own environment. Our DSPs help with daily living skills, community access, social engagement, and personal care — always following the individual’s lead about what support looks like for them.
Community integration doesn’t happen inside four walls. Hope Hearth provides reliable transportation for medical appointments, day programming, employment, shopping, social outings, worship, and anywhere else life takes you. Our drivers are trained in supporting individuals with IDD and treat every trip as an opportunity for community participation, not just a ride from A to B.
